When I use word, I would like the left mouse button to enter. (I got that part figured out)
But if I want to enter in the middle of the line, how do I do that?

Not entirely sure what you mean?
If you use {return} or {enter} in simulated keys (I presume that's what you are talking about?) it will simulate that key whenever you press the mouse button - it has no concept of the application with keyboard focus (Word?) it just sends the 'enter' key when you press the mouse button... so its up to you when you press the button?
If you are typing more than one key, a sentence as such, you could do:
"this is a{return}test"
or something, but I'd have thought that was obvious so probably not what you are getting at?!
The only other thing I can think you mean, is working out the content of the line, moving the cursor to a defined point, and then pressing enter. That's basically not possible with XMBC, because as I say, XMBC has no concept/access to the text (or anything else) in the application that has keyboard focus.
You wont need to have some macro in Word (in this case) that can work out where the return needs to be placed. You may be able to trigger that macro from XMBC, by sending a hotkey combo that would then run the Word macro (Im assuming word macros are still a thing here - haven't used them myself for years!)
