Side game mouse thumb button to be shift and work with scroll wheel button

What do I need to setup in the Xmouse Button Control Software to get my shift key(keyboard button) and my Gamestart Gaming mouse to work together. I need to set one of my side thumb buttons to be the shift key, which I have done and my scroll wheel button to work together. In Fusion 360 this allows me to orbit a sketch. I have tried a couple of different things but just not getting it.

Hi, I'm not entirely sure what you are asking, you say you have done the shift key on button 4, I presume using simulated keystrokes?
But how do you want it to interact with the scroll wheel? In simulated keystrokes you can choose how to send the key, by default (method 1) will press and release it as soon as you click the button, so it will not hold the key down while to hold the button down (which I guess is what you want?).
You need to choose method 3 (during) to make XMBC hold the key (shift) while the button is down, thus any other interactions with the mouse/keyboard will behave as if shift is held.

So first is to try: Simulated keystrokes, {shift} and send Method 3 (during).
If that is not what you are asking, then apologies, but can you please try and be a little more specific?
