Windows Version: 10
Mouse Information (brand/model): 3M Ergonomic Mouse
Relevant Computer Information (CPU, RAM etc): N/A
Did the problem occur after an upgrade of XMBC? (If so, from what version?): N/A
Did the problem occur after a Windows update/upgrade? (If so, from what version?): N/A
How long have you used XMBC?: 1 week
What language and keyboard layout do you use in Windows?: English

Clear description of the problem - try and include as much information as possible, including what button and mappings you are having problems with (if applicable).:

I currently have physical right click bound to simulated middle click and would like to extend this to support scroll wheel emulation through a click and drag motion.

I.e.,

While holding right click (now bound to middle click), dragging up and down on the Y axis should respectively scroll the "wheel" by some tick up and down.

Is this currently possible in the application? If so, how?

It should be possible, if you set the right button to "button held" which allows different actions based on how long the button is held for.
You can specify the delay (time required held) for up to 3 actions.
you can then select Movement to Scroll for the action to apply after x milliseconds of holding the button.

NOTE: It is probably wise to make sure that the movement to scroll is the last held action (if you have more than 1) as otherwise, it will kick in and stop the button held timer (I think).

Oh I haven't personally thought of everything, but there are many before you here who had had their influence on it over the years.
Glad it worked - I like requests like that where I don't need to make any further changes - they are nice and quick :)
