OK I got a M650 and did some tests.
Firstly, the 4th and 5th buttons have a built in "chording" type functionality by default, where you hold button 4 or 5 and scroll to horizontal scroll (yuck).
When this is enabled, buttons 4 and 5 are not visible to XMBC (as people have suggested)
But this can be disabled in the Logi options+ software, under Point and Scroll - doing so makes XMBC see button 4 and 5 again (assuming they are mapped to forward and back in options+ button mapping).
However, if you uninstall Logi Options, it will revert to "factory" defaults where holding button 4 and 5 turn the wheel into horizontal scroll - so unfortunately you need to leave Logi options installed (Im not sure if it needs to be running though?!)
