"Previous Window" problem
Previous window has a small problem for me. It does not select the previous window. I just goes through all the open windows, one by one. So If I have 5 open windows and I select "previous window", I have to hit the button 5 times for the real previous window to come around. If I only have 2 windows open, It will work correctly, but I have to hit the button twice. Thanks for reading and this software is awesome for my mx500 mouse in Windows 7-64.

That's exactly how it should work. I'm pretty sure Previous Window works by sending an ALT-ESC, which drops the current active window to the bottom of the z-order stack, making the last selected window the new active window. It does this all the way through the whole pile of non-minimized windows, going through each and every previously selected window.
If you want to swap between the 2 top-most z-order windows with a single button you'd have to send an ALT-TAB not ALT-ESC.
EDIT: XMBC's ALT-TAB won't work though, that's actually a CTRL-ALT-TAB which locks the ALT-TAB dialog on-screen. You want to send the standard-plain-old ALT-TAB command. I don't know if it's possible to completely suppress the ALT-TAB dialog though, it always flashes on-screen for a split second.

For the cleanest presentation you'll want to use the following alternative for the Vista/7 ALT-TAB...

It is a HIGHLY customizable replacement for the ALT-TAB function. Play around with it's many settings to see what presentation you prefer, it can be configured to swap between two windows without an on-screen dialog. If you'd like more advice on how best to set it up for the functionality you desire and getting it to work through X-Mouse let me know!

Simulated Keystrokes does work. But I have to hit the button twice. That is moderately annoying.

No, you don't have to press twice.
Assign the following Simulated KeystrokeYou can paste that directly into XMBC.
and selectfrom the combo box directly beneath it.

I certainly appreciate your help Maintrane. The only thing I didn't like about it was the little "app switcher"

That's why I suggested using Switcher 2.0; it has a much smoother transition.

Using Switcher 2.0 to do Active Window Swapping
General Settings
Views > Pick 'Tile'
Check 'Automatically start at logon'
Appearance Settings
Show the desktop 'in the background' (50% brightness recommended)
Window Style Settings
Uncheck all options for both normal and selected (you can optionally put a semi-transparent black frame over normal non-selected windows to help identify which window will be activated)
Shortcuts Settings (most important part here)
EDIT SHORTCUTS, locate the following, SESSION > BEGIN AND SELECT NEXT
Choose you're preferred hotkey activation method(s), I use ALT-F5. Preferably something that doesn't conflict with any other hotkeys.
Check 'Global shortcut'
Activate when shortcut is 'pressed|released', take your pick.
Check 'End session when shortcut is released'
You can then assign this hotkey to a mouse button with XMBC (choose anything but an as button is pressed option)
Switcher is a very robust ALT-TAB alternative, so you will find a lot more useful applications (not just active window swapping) if you delve into it.
That's why I suggested using Switcher 2.0; it has a much smoother transition.
