First off, thank you for such a useful piece of software!
In this new version, simulated keystrokes mapped to the 4th and 5th buttons do not work with application-specific settings. Simulated keystrokes only work with the default profile.
I'm using Vista 64 bit. I rolled back to an earlier version, 1.34, and simulated keystrokes work just fine in application-specific settings.

Oh no - have I screwed up.. And I thought I tested it so very well

OK, your right, its broken in 1.45
It is also fixed in 1.46 which is now active for download.
