First I want to thank you for your amazing program. Thank you very much.
And I want to talk about one problem that I have. I use a program which named "Adobe Illustrator CS6". When I enter the keyboard shortcut Ctrl+C on the button 4 and Ctrl+V on the button 5 this not works right. Program sends the shortcuts without Ctrl. I am getting symbol C or symbol V only. That is instead of copying and pasting in the text I see "C" or "V".
I have Win7 at home and at work. And I have different mouses but the same problem. So I took mouses of my colleagues and saw the same bug.
This is a big problem for me. But I hope you can fix it.

I tryed to set same as yours for Adobe Potosop elements 6 on XP. Ctrl+C/V worked properly. But using XMBC ("Copy/Paste" for the button 4/5) with Setpoint software (Back/Forward), copy/paste actions fault. Then I tryed to set the actions of the button 4/5 into "Simulated Ketstrokes"
Thank you very much guys but it doesn't work in adobe illustrator only. I haven't problems with other programs of adobe (Photoshop and InDesign). It's sad but all of your tips didn't work in illustrator.

What mouse do you have?
Do you use other drivers/programs for your mouse (like logitech setpoint or razer synapse)? [I recommend you to uninstall all, so only XMBC works]
You can use copy and paste in two ways:
1. as simulated keystrokes - then you write {ctrl}c and {ctrl}v
2. as actions prepared by author - scroll the list with all actions down to the "copy (ctrl+v)" and "paste (ctrl+v)"
Which way do you use?
Let the PC do the hard work = AHK + C# + HTML/CSS/JS + XMBC + Clavier+ + AutoLisp + VBA/OOB

Hi. Good wishes from Ukraine to dear polish brother. I have a mouse without special drivers. The manufacturer of the mouse is CBR, model CM 515. But I tried other mouses also. Im sorry but Your propose not works.
