Stranger still... If you copy the path from the log (without the filename) and paste it in to explorer, does it take you to the right/valid place? Interesting about UAC, I don't think I've ever tested XMBC with it turned off (turned down perhaps but not off completely) - I wonder if that's related!....

If I recall correctly, there is a long standing bug in the long call inside the driver itself. I can not fix it as I don't have the ability to sign the driver even if I wanted to! Thats odd - its working here, at least it works for volume UP. I have found though that volumne down (using the keyboard key and XMBC) gets accelerated after 4 or 5 nothes then it shoots down really quick (does not do this going up). Well that was a complex one to diagnose - its difficult to debug something before you logon 
Anyway I think Ive found the problem and worked around it. 2.20 Beta 5 should fix this (available in the beta topic here) if your interested in testing it!

Start > Settings > Accounts > Sign-in options > Use my sign-in info to automatically finish setting up my device and reopen my apps after an update or restart. It IS an XMBC bug. It worked FINE before 2.20 beta 4.
2.20 Beta 4 included a "fix" for smooth scrolling which actually broke this. Its a known problem that I hadn't gotten around to fixing.
Ive fixed it now so beta 5 will be all good again.
