XMBC can control the windows mouse pointer already to do this, but I very much doubt it will ever be able to control games in that way. This is not possible. Simulated keys works by sending to the keyboard buffer, so the output of that is directed, by Windows, to the active window. I mean, it could continue sending keys, but they would not go to the game, they would go to what ever screen has focus, no different to if you were press...

Yes it is intentional. because if you are using button held, then it was to prevent you choosing a sticky where it could confict with the button still being held down. That seems to be working here for me in the latest beta, maybe you could try that.
I've also modified the latest so that it will highlight the buttons for injected/simulated button presses (in a different colour (light red)) so it should be more obvious something is going on.
