This isn't a problem, and it seems obvious, but wanted to ask in case my obvious wasn't the same as someone else's...

I can write {ctrl} {CTRL} and {CtRl} all interchangably, right?

and for normal keys, upper or lower case also makes no difference, so I can't
type a capital X to get a raised X pressed there, I need to explicitly use {shift}X or {Shift}x (or similar), right?

Yes correct. The Simulated keys is just like hitting the physical key on the keyboard (normally)... So if you want a captial letter you have to use SHIFT + the letter {shift}x as you say.

The tags in XMBC should be case insensitive - i.e. it does not matter if you use upper/lower or a mix of both. If I recall correctly, XMBC converts the string to lower case anyway when processing it
